I have READ and Understand that EVERYONE has to be Vaccinated to use the Hall, as noted below.*
As we are in RED Level NO gatherings can occur in the Hall for anyone who is unvaccinated. We need to get your assurance (IN WRITING) that no one will be in the hall that is unvaccinated. In terms of Occupational Health and Safety standards we have to follow the rules, especially as we are a Church and must be seen to lead by example. YOU AGREE that you will only allow people with a vaccine pass and that YOU will be responsible for checking the passes. Max number is 100 Fully Vaccinated people.
Myself or anyone using the hall is NOT associated to any Patched Gang Members.*
As we have damage done to our property when Patched Gangs members have hired the hall we will not hire to Patched Gang members or anyone associated with Patched Gang members. Thank you for your understanding in this regard.

Agreement In Writing

RED LEVEL Restrictions on Numbers - You understand you cannot exceed 100 fully vaccinated people in the Hall.

We need to get your assurance (IN WRITING by acceptance of this form) that no one will be in the hall that is unvaccinated.

In terms of Occupational Health and Safety standards, we have to follow the rules, especially as we are a Church and must be seen to lead by example.

YOU AGREE that you will only allow people with a "My Vaccine Pass" and that YOU will be responsible for checking the passes for all those that are to use the Hall.
